A clinic first needs to be accessible to the system. It then needs pages and public sources that match the patient's location and question.
From there, the assistant retrieves information and builds an answer. The exact source mix can change. No individual signal guarantees that the clinic will be named.
There is no universal AI recommendation score
OpenAI does not publish a complete ranking formula for local clinic recommendations. Google and Microsoft have their own search systems. Each product can use different models, indexes, search partners, and user context.
Anyone claiming that one schema type, review count, or article format guarantees recommendations is offering certainty the platforms do not provide.
Serum uses a working model. We label what the platforms confirm. We treat the rest as an interpretation to test.
What the platforms confirm
ChatGPT may search and rewrite the question
OpenAI says ChatGPT may search the web automatically when a question would benefit from current information. The system may rewrite a prompt into one or more targeted searches and can use approximate location.
“Where should I get Botox?” becomes a different retrieval problem once a city, neighbourhood, or provider requirement is added.
Google's AI features draw from the search index
Google says a page must be indexed and eligible to appear with a snippet before it can appear as a supporting link in AI Overviews or AI Mode. Google may use query fan-out across related subtopics and sources.
Google also says there is no special schema or separate AI file required. Its existing SEO fundamentals still apply.
Bing connects Copilot visibility to search
Microsoft says Bing and Copilot search experiences rely on the same core crawling, indexing, and ranking foundation as traditional search.
Its guidance calls for discoverable URLs, accurate information, focused pages, and clear entity definitions. Structured data may help grounding, but Microsoft does not guarantee visibility or citations.
A practical model for how a clinic reaches the answer
Stage 1: The assistant interprets the patient question
The prompt sets the job. A patient may ask for a treatment, a nearby clinic, a comparison, a budget, or a provider with a certain credential. A clinic cannot control that context. It can make its own location, services, and provider information easier to understand.
Stage 2: The system needs eligible sources
A source cannot help if the relevant system cannot access or index it. OpenAI, Google, and Bing use their own crawlers and search stacks. A security layer can block a crawler even when robots.txt looks fine.
Stage 3: The system retrieves pages that match
A generic homepage has to cover the whole business. A focused page can explain one treatment in one location with more useful detail. Titles, headings, internal links, and readable body text help establish that focus.
Stage 4: The system has to make sense of the clinic
Does the website use the same clinic name and address as the public profile? Are providers named? Do service and booking pages describe the same offering? Bing explicitly recommends clear and consistent entity definitions.
OpenAI does not confirm a clinic-consistency score. Serum treats corroboration as a working interpretation because conflicting facts make any source harder to use.
Stage 5: The assistant builds a response
The assistant may mention several clinics, cite supporting pages, leave out a relevant clinic, or use stale information. AI visibility work improves the evidence available. It does not give an agency control over the final wording.
What can influence visibility in practice
| Area | What to make clear | What it does not prove |
|---|---|---|
| Location | Clinic address, service areas, and the location attached to each service | That the clinic is the best option nearby |
| Treatment relevance | A focused page that explains the offering and next step | Clinical suitability for one patient |
| Provider information | Real names, roles, and verifiable credentials | Clinical superiority |
| Public reputation | Accessible review profiles and accurate claims | That a review count guarantees a mention |
| Technical clarity | Crawl access, indexable pages, internal links, and accurate schema | That compliance forces placement |
Reviews deserve their own boundary. Google confirms value in local search, while OpenAI publishes no review threshold for clinic recommendations. See the full med spa reviews and ChatGPT evidence check.
What verified professional profiles should mean
It should not mean stock headshots and vague titles. A provider page should name the real person and state their current role. Any licence, certification, or membership should be current and verifiable.
For Serum, the same principle applies. Nadir's founder profile should link to a real professional presence. Specialist collaborators should not be presented as employees when they are not.
Where SEO, AEO, and GEO fit
SEO helps search systems discover, index, and understand a website. AEO focuses on making accurate answers easier to retrieve. GEO is commonly used for work aimed at visibility in generated responses and citations.
Google says standard SEO fundamentals still apply to AI search. Microsoft uses GEO in its guidance but does not guarantee grounding. GEO should extend sound search work, not replace it.
If you are deciding where to invest first, use our comparison of AI visibility and local SEO for med spas.
Search Atlas estimated 4,400 US monthly searches for “generative engine optimization” with a pressure score of 54 on August 21, 2026. “What is generative engine optimization” showed 880 and a pressure score of 72. Exact med spa variants showed zero recorded volume. These are platform estimates, not traffic promises.
What a clinic owner should do next
- Write down the patient questions closest to a booked consultation.
- Run the same prompts with explicit city and treatment context.
- Save the named clinics and cited sources.
- Check whether the important pages are accessible.
- Compare the facts and evidence available for your clinic.
- Fix the smallest verified gap, then repeat the test.
Frequently asked questions
Does ChatGPT use Google rankings to choose clinics?
OpenAI says ChatGPT search may use third-party search providers, but it does not publish a rule that copies Google's rankings. Treat visibility in each platform as its own result.
Are reviews a confirmed ChatGPT ranking factor?
OpenAI does not publish a fixed review-count formula for local clinic recommendations. Reviews are useful public evidence, but no threshold guarantees a mention.
Does schema make a clinic rank in AI answers?
Schema can give supported systems machine-readable facts. Use accurate markup as a clarity layer, not a shortcut or guarantee.
Why can the answer change when I run the prompt again?
The system may use different search queries, sources, location context, or conversation context. The available web information can also change.
