Serum Agency is responsible for the personal information described in this notice. Questions or deletion requests can be sent to [email protected].
What we collect
When you request a free clinic snapshot, we collect your name, work email, clinic name, website, city, priority treatment, number of locations, and any optional competitor you provide.
We also record the page you submitted from, the referring page, campaign parameters in the link, the consent wording shown, and the date of the request. If you book a call, Calendly provides the booking details you enter there.
Do not send patient names, health information, treatment records, private photos, passwords, or portal credentials. The free snapshot does not need them.
How we use it
- To research, prepare, and deliver the snapshot you requested
- To respond to your inquiry and manage any later sales conversation
- To prevent duplicate work and keep the Serum Master CRM accurate
- To understand which pages and campaigns generate qualified interest
- To send optional marketing email only when you choose that separate checkbox
Where it is recorded
Snapshot requests are recorded in the existing Serum Master CRM in Google Sheets. The website sends the request through a Cloudflare Worker and a Google Apps Script bridge. The form does not put CRM credentials in the browser.
Booking data is handled by Calendly under its own privacy terms. Website infrastructure may be provided by Cloudflare. Search Atlas may process public page content for website optimization. We do not sell your personal information.
Website measurement
Serum records necessary, first-party funnel actions such as a relevant visit, snapshot form start, form submission, booking click, and completed booking. This operational measurement is used to run the requested service and keep the Serum Master CRM accurate.
Google Analytics is optional. It loads through Google Tag Manager only after you accept analytics cookies. Analytics receives the page path, event name, and CTA location. It does not receive names, email addresses, clinic names, website domains, competitor names, or form answers. Advertising pixels are not currently active.
You can accept analytics, use necessary tools only, or change your choice at any time through the Cookie choices link in the footer. Your choice is stored in this browser.
Marketing consent
Permission to deliver a requested snapshot is separate from permission to send marketing email. The marketing checkbox is optional and is not required to receive the report.
You can unsubscribe from marketing messages at any time. Transactional messages needed to deliver the requested report may still be sent.
Retention and deletion
We keep inquiry and CRM records only as long as needed to deliver the request, manage the business relationship, maintain an accurate history, or meet legal obligations. You can ask us to correct or delete personal information by emailing [email protected].
Security
We use access controls, server-side validation, bot protection, and scoped service credentials. No system is perfectly secure, so we collect only the information needed for the stated purpose.
Changes
If the collection or use of information changes in a material way, we will update this notice and its effective date.
